Information: Provides referrals to mental health services, including case management; this is the gateway agency to mental health services in Cuyahoga County. Offers two programs that provide case management, links to housing services, and ongoing mental health, employment and substance abuse services to formerly incarcerated persons who are homeless. The Women’s Forensic Program is for homeless women with co-occurring substance abuse and mental health disorders released from The Cuyahoga County Jail up to 30 days after release. The Returning Home Ohio Program is for those with a mental health disorder or HIV and up to 120 days after release from a corrections facility.
